DaVo and his staff will not pierce anyone under the age of 18 without a parent / legal guardian present and willing to fill out and sign the piercing waiver or a notarized letter of consent. This is not because an Iowa State or Local law prohibits or regulates the piercing of minors, but rather for ethical reasons and liability risks. Under Federal law, a minor can not enter into a legally binding agreement and thus can not sign a waiver consenting to be pierced.

We require one of the following to pierce anyone under the age of 18:

• A parent is present to sign a consent form. If the minor has a court-appointed guardian, we require a photocopy of the court order.
• Suppose the parent or legal guardian can not be present at the time of the piercing. They can stop by, fill out, and sign the consent form in advance.  
• If the parent or legal guardian cannot be present at the time of the piercing or stop by to fill out and sign the consent form in advance, we require a notarized letter of consent. Meaning that the guardian writes a letter to consent the minor to be pierced and then signs in the presence of a Notary. Thus providing third-party verification that the minor's guardian gave consent. • If the minor has been emancipated, we require a photocopy of the emancipation proclamation.
